**Job Description:**
- Ensure prescreen is filled out and attached upon submitting a candidate.
- Attach resume and license verification for submission.
- Must pass a credentialing checklist prior to hiring.
- Participate in therapy sessions and create a supportive environment for patient rehabilitation.
- Collaborate with other healthcare professionals to develop treatment plans.
- Maintain detailed and accurate patient records.
- Adhere to all facility policies and procedures.
- Conduct appropriate patient assessments and modify treatments as necessary.
- Promote patient education and involvement in their own recovery process.
