**Job Description:**

- Provide physical therapy services to patients in their place of residence suffering from physical impairments, functional limitations, or disabilities due to injury, disease, or other causes.
- Collaborate with patients, their families, physicians, and other allied health professionals to identify patient needs and coordinate services.
- Utilize thorough and timely electronic documentation and interdisciplinary communications in accordance with agency policies, procedures, and standards of practice.
- Work with a diverse patient age range from birth to over 100 years old.
- Participate in weekend rotations once every six weeks.
- Ability to travel to assigned cases as needed.
