Job Description:
- Provide physical therapy services in a home health setting.
- Work collaboratively with home health and hospice teams to develop patient care plans.
- Evaluate and assess patient's physical conditions, develop treatment plans, and set goals based on the patient's individual needs.
- Implement treatment plans using various techniques and modalities to help patients regain mobility and manage pain effectively.
- Monitor patient progress and adjust treatment plans as necessary.
- Ensure all documentation is compliant with healthcare regulations and standards.
- Communicate effectively with patients, family members, and healthcare team members to ensure optimal patient outcomes.
