**Experience:** Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience

**Certifications:** CPR, BLS

**Must-Have:**
- Reliable transportation, valid driver's license, and required liability insurance
- Proficiency in HomeCare HomeBase (HCHB) and OASIS, with use within the last 6 months

**Job Description:**

- Provide, coordinate, and direct the provision of home nursing care based on physicians' orders and agency policies
- Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses
- Conduct home visits and periodic office meetings
- Manage an average of 5-8 patients per day, achieving 30 productivity points per week
- Handle various types of cases such as Start of Care, Wound Care, and Follow-up Visits
- Responsible for completing all documentation within 24 hours of patient appointment
- Willing to travel up to 75 miles from the home clinic; mileage reimbursed at IRS Standard rate
- Open to floating to nearby clinics to meet required hours if patient census is low
- Dress code requires scrubs
