**Job Description:**

- Perform evaluations, diagnostic testing, and treatment of patients as prescribed by a referring physician.
- Meet all Medicare program requirements for physical therapists.
- Demonstrate knowledge and ability to manage patient rehabilitation plans effectively.
- Exhibit excellent communication skills and professional behavior.
- Maintain comprehensive and accurate patient records.
- Collaborate with other healthcare professionals to ensure coordinated care.
- Provide guidance and support to patients for sustainable health improvements.

This role is based primarily at our Wellness Center location and requires normal urgency. Holiday coverage is not required.
