**Must-Have:** Bachelor's Degree with completion of an accredited program in PT

**Job Description:**
- Conduct evaluations of patient/resident status and needs.
- Plan treatment and establish realistic, measurable short-term and long-term goals with the referring provider, patient/resident, family/caregivers, and team members.
- Provide and evaluate the effectiveness of therapy treatments and plans in accordance with department policies and procedures.
- Educate patients/residents, family/caregivers, and team members to enhance the attainment of established goals.
- Deliver care/treatment suitable for the patient's/resident's age, physical ability, and intellectual development.
- Complete accurate, legible, timely, and effective documentation as per department policies and procedures.
- Communicate clearly and concisely with team members, the referring provider, and insurance/case managers about patient/resident status, progress, needs, and discharge planning.
- Delegate appropriate patient/resident treatments to therapy assistant staff and provide supervision and direction to therapy assistants.
- Complete reevaluations with therapy assistants as indicated and according to department policy and procedure.
- Complete daily billing accurately.
- Schedule patients efficiently in cooperation with receptionist.
- Participate in department and hospital-wide educational opportunities for continuing professional development/education.
- Provide one department in-service if attending a continuing education course reimbursed.
- Participate in the Rehab Department student program as available and appropriate as a clinical instructor, mentor, and role model.
- Participate in one Rehab Department or project, PI initiative, or committee.
- Adhere to all safety policies and procedures. Report incidents, falls, adverse drug reactions, and equipment issues according to policies.
- Participate in the development of and adhere to the department's Mission, Values, and Ground rules. Embrace cultural changes and implementation.
