**Job Description:**
- Provide physical therapy services to patients in accordance with their treatment plans
- Conduct patient evaluations and formulate individualized treatment objectives
- Collaborate with medical professionals and caregivers to assess patient progress
- Document patient progress and modify treatment plans as necessary
- Maintain compliance with federal, state, and professional regulations
- Engage with interdisciplinary team members for comprehensive patient care
- Demonstrate excellent communication and interpersonal skills
