**Job Description:**

- Ensure that all prescreen documents are completed and attached when submitting the candidate.
- Attach resume and license verification to the application.
- Conduct a COVID test 72 hours prior to the start date, with results provided 48 hours prior.
- Complete credentialing checklist items including:
- State license verification
- Resume and three references
- County, federal, and multi-state criminal searches (covering the last seven years)
- Nationwide sex offender search (covering the last seven years)
- HHS/OIG and GSA searches
- Social security number trace
- 2-Step PPD or Quantiferon test, or proof of positive PPD and chest X-ray
- SAM search
- Facility-run Oregon State background check (CRIMS)
- Normal urgency for position
- Bilingual not required
- No holiday coverage required
