**Job Title:** Physical Therapist

**Job Specialty:** Physical Therapy - Home Hospice

**Job Duration:** 13 weeks

**Shift:** 8-Hour Days, Monday to Friday

**Guaranteed Hours:** 40 hours per week

**Experience:** Open to new graduates and first-time travelers

**License:** SD licensure

**Certifications:** BLS, National accreditation through NPTE

**Must-Have:**
- General physical therapy skills including evaluation, intervention, and care plan development
- Ability to work with medically compromised patients, including those with CHF, COPD, DM, cardiac issues, orthopedic, neurological conditions, dementia, and general weakness
- Experience with patients ranging from early adult to geriatrics

**Job Description:**

- Provide physical therapy services to patients in hospice home settings.
- Participate in the evaluation and development of effective intervention strategies.
- Conduct medical assessments and develop personalized care plans for each patient.
- Work collaboratively within a multi-disciplinary team including RNs, LPNs, OTs, PTAs, SLPs, aides, and case managers.
- Utilize the EPIC charting system for documentation and patient records.
- This role involves traveling to various locations to provide patient care in the Southern Black Hills and surrounding areas.
- Attend one week of orientation, including EPIC system training, shadowing, and documentation processes.
- Maintain professional attire, with the option to wear any color of scrubs or business casual clothing.

**Other Notes:**
- No weekend rotation or on-call obligations.
- Office is closed on major holidays.
- Approval for time off requires manager approval.
