**Certifications:** Valid Driver's License, Proof of personal auto insurance, BLS required
**Must-Have:** Strong computer skills, Ability to work independently, Excellent communication and customer service skills
**Job Description:**
- Responsible for evaluating, planning, directing, and administering Physical Therapy services as prescribed by a licensed provider.
- Provide treatments to restore function, improve mobility, relieve pain, prevent or limit physical disabilities, and increase independence to enhance patients' overall functioning.
- Treat patients using Physical Therapy modalities and skilled therapy techniques; instruct patients in the use of adaptive or assistive equipment.
- Supervise and direct Physical Therapy Assistants; maintain open communication with Rehab team members and consult other departments collaboratively for positive clinical outcomes.
- Perform appropriate assessments on all patients related to physician's orders; interpret results within the scope of licensure.
- Design treatment programs for patients with varying disabilities and multiple medical problems according to accepted practices and physician orders.
- Formulate realistic short and long-term treatment goals including patient and family involvement in care plans.
- Observe and evaluate treatment effects, recommending changes to the physician as necessary.
- Ensure accurate and timely patient charges, documentation, and compliance with regulatory standards.
- Provide information related to department budget concerning Physical Therapy needs for optimum patient care and service.
- Maintain timely, efficient documentation meeting current standards and policies, demonstrating thorough knowledge of computerized documentation including evaluations, progress notes, monthly notes, and discharge summaries.
- Perform all aspects of the role in an environment optimizing patient safety and reducing healthcare errors.
- Comply with established safety procedures ensuring a safe environment for patients, visitors, and staff.
- Participate in performance improvement activities.
- Perform other assigned duties as needed.
