Travel Physical Therapist (Pt) In Santa Rosa, Ca

**Job Title:** Physical Therapist (PT) - Home Health

**Shift:** Day Shift, 5x8-Hour (08:00 - 16:30)

**Guaranteed Hours:** Work Week as per contract

**Experience:** Minimum 1 year of skilled PT experience (no prior home health experience required)

**License:** Pending license accepted

**Certifications:** Basic Cardiac Life Support (BCLS) required

**Must-Have:** Reliable transportation

**Job Description:**

- Provide home health physical therapy services in designated areas
- Day shift with up to 6 patient visits per day; 30 productivity points standard
- First-time travelers not accepted
- One weekend shift required per contract
- No on-call duties required
- Floating between like units is not required
- Not open to block scheduling
- Occasional maximum travel radius of 50+ miles; efficient client grouping to minimize drive time
- Familiarity with charting system preferred, but not required
- Orientation consists of 40 non-billable hours
- Completion of non-billable modules pre-start and annually

Note: Compliance with travel guidelines, including floating arrangements, is expected. Additional requirements include a minimum of two supervisory references covering one year of employment within a comparable specialty/setting in the last three years.
