Travel Physical Therapist (Pt) In Santa Rosa, Ca

**Shift:** Day 5x8-Hour (08:00 - 16:30)

**Guaranteed Hours:** Full work week per contract

**Experience:** 1 year of skilled physical therapy experience required

**License:** Pending license accepted

**Certifications:** BCLS

**Must-Have:**

- Own transportation required
- Oasis experience preferred, but not required

**Job Description:**

- Provide home health physical therapy services in specified areas
- Work an 8-hour day shift; facility cannot accommodate 4x10s or 3x12s
- Maintain a 1:1 patient ratio, visiting up to 6 patients per day
- Meet 30 productivity points standard
- Weekend shift required once per contract
- Chart using EPIC system
- Mileage reimbursed per IRS standard
- Must have reliable transportation
- Occasional travel within a maximum radius of 50+ miles
- Complete non-billable modules, average 16 hours spent
- Participate in 40 hours of orientation

- Not open to first-time travelers
- Not open to block scheduling
- Local candidates within a 50-mile radius accepted at the same rate
- COVID-19 vaccine required, with medical or religious exemptions allowed
