**Must-Have:**
- Primary series COVID vaccination in addition to one booster
- Reliable transportation
- Willingness to float between all facilities within a given state

**Job Description:**
- This position involves providing home health physical therapy services in Seaside and surrounding areas.
- Expectation to visit up to six patients per day with 30 productivity points as a standard.
- No weekend or on-call requirements.
- The system efficiently groups clients to minimize driving and travel time. Occasional maximum radius of roughly 60 miles per day.
- Use of EPIC Remote Client for Home Health and Rover for charting.
- Business casual attire; scrubs are allowed but not mandatory.
- Facility is open to 26-week contracts.
