Travel Physical Therapist (Pt) In Seaside, Or

**Job Title:** Physical Therapist - Home Health

**Job Specialty:** PT Home Health

**Job Duration:** 13 weeks (with potential for extension to 26 weeks)

**Shift:** Day 5x8-Hour (08:00 - 16:30)

**Guaranteed Hours:** Facility can call off up to one shift per pay period (every two weeks)

**Experience:** 1 year of skilled home health experience required

**License:** Pending license accepted

**Certifications:** Basic Cardiac Life Support (BCLS)

**Must-Have:**
- Reliable transportation
- Primary series COVID vaccination in addition to one booster

**Job Description:**
- Provide skilled home health physical therapy services in Seaside, OR, and surrounding areas.
- Work schedule includes 8-hour day shifts; cannot accommodate 4x10s or 3x12s.
- Patient care ratios are 1:1, with potential for up to 6 patient visits per day.
- Meet a standard of 30 productivity points.
- The facility accepts first-time travelers and locals within a 50-75 mile radius.
- No requirement for on-call, weekend shifts, or floating between units.
- The system will group clients to minimize drive and windshield time, with a maximum daily travel radius of roughly 60 miles (60-90 minutes).
- Business casual attire recommended; scrubs allowed but not required.
- Utilize EPIC Remote Client for Home Health and Rover for charting.
- No cost communication devices to be provided, or reimbursement for work-related cell phone use.
- Complete non-billable modules pre-start and annually; anticipated average of 16 hours.

**Orientation:** 40 hours
