**Job Description:**
- Ensure prescreen is filled out and attached upon submitting candidacy.
- Attach resume and license verification; candidates will be declined if these items are not completed.
- Conduct a thorough background check, including:
- State License Verification
- Providing three references
- County Criminal check for the past 7 years for all counties lived and worked
- Federal Criminal check for the past 7 years or more
- Multi-State Criminal Search for the past 7 years or more
- Nationwide Sex Offender Search for the past 7 years or more
- HHS/OIG Search
- GSA Search
- Social Security Number Trace
- 2 Step PPD or Quantiferon or proof of Positive PPD and Chest X-Ray
- SAM Search
- Oregon State Background - CRIMS (facility run)
- Bilingual and holiday coverage not required.
