**Job Description:**
- Ensure all required pre-screen documents are submitted.
- Prefer experience with Rehab Optima.
- Complete a comprehensive credentialing checklist including:
- County, Federal, and Multi-State Criminal Checks (7 years or more)
- Nationwide Sex Offender Search (7 years or more)
- HHS/OIG and GSA Searches
- Social Security Number Trace
- SAM Search
- Physical examination

- Participate in NABS and Fingerprinting conducted by the facility.
- Orientation involves reviewing patient care protocols and compliance standards in a dementia-focused environment.
- Maintain a professional and compassionate attitude towards patients needing physical rehabilitation services.
