**Job Title:** Physical Therapist - Home Health

**Shift:** Day 3x12-Hour (07:00 - 19:00)

**Guaranteed Hours:** Based on hours signed

**Experience:** 2 years preferred; will consider 1 year on a case-by-case basis

**License:** Will hire with a pending license; must obtain prior to starting

**Certifications:** BLS required

**Must-Have:** PT Home Health experience

**Job Description:**

- Provide physical therapy services within a home health setting.
- Work shifts of either 4x10s Friday-Monday or 3x12s Friday-Sunday.
- Participate in an orientation including a 16-hour non-billable training.
- Expected to float within the scope to meet facility needs.
- Must pass a Cardiac Dysrhythmia Test during orientation.
- Participate in a pass/fail test on Cardiac Dysrhythmia and, if applicable, Moderate Sedation.
- Submit compliance documents 5 business days before the start date.
- Must travel within a 75-mile radius.
- All requested time off must be included in the initial submission.
