**Must-Have:** Experience with Home Health

**Job Description:**

- Restore function and prevent or minimize disability following disease or injury.
- Help clients reach their maximum performance and function within their capabilities.
- Evaluate and treat patients using medically prescribed physical therapy programs.
- Supervise Physical Therapy Assistants as needed.
- Conduct initial admission/evaluation visits.
- Notify the Clinical Team Manager of OASIS scores within established timelines.
- Document observations, clinical findings, problems, skilled interventions/treatments, goals, and discharge plans.
- Perform other duties consistent with skills and training, aligned with company goals and mission.
- Service territory includes Milpitas, Saratoga, Monte Sereno, Los Gatos, Campbell, San Jose, Morgan Hill, San Martin, Gilroy, Coyote Point, Sunnyvale, and neighboring cities as needed.
- Use the Homecare Homebase HCHB electronic medical record system.

**Note:** This position does not require holiday coverage. Bilingual skills are not required.
