**Job Title:** Physical Therapist

**Shift:** Day 5x8-Hour (08:00 - 16:30)

**Guaranteed Hours:** Client may call off up to 36 hours per thirteen-week assignment without paying for the canceled hours.

**Experience:** Minimum 1 year experience required. First-timers accepted.

**License:** Active IL state license required; pending applications with ETA considered.

**Certifications:** Basic Life Support (BLS) required; Flu Vaccine required (medical/religious exemptions only).

**Must-Have:**
- Assessment Skills: AROM/AAROM/PROM, Arousal and Attention, Body Mechanics, Identifying Environmental Barriers, Pain Assessment, Skin Integrity, Strength Testing, Vital Signs
- Documentation Skills: Assessment, Discharge, Evaluation, Long Term Goals, Plan of Care, Reassessments, Short Term Goals, Time Management
- Equipment and Device Skills: Gait Belt
- Professional Skills: Home Exercise Program, Patient and Family Education, Recognizing Signs & Symptoms of Physical, Sexual, Psychological Abuse, Safety, and Injury Prevention

**Job Description:**
- No on-call duties required.
- No floating within the scope to meet facility needs.
- Block schedule for Monday to Friday.
- Must work 2 out of 3 major holidays.
- Modules to be completed virtually on first day and annually thereafter.
- Required to follow charting procedures using Cerner software.
- Orientation includes 16 hours non-billable.
