**Must-Have:** Proficiency in HomeCare HomeBase (HCHB) and OASIS; used within the last 6 months
**Job Description:**
- Provide, coordinate, and direct home nursing care according to physicians' orders, based on agency policies and procedures, through the competent application of the nursing process.
- Responsible to the Clinical Manager.
- Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Engage in home visits and attend periodic office meetings.
- Require reliable transportation, a current driver's license, and agency-required liability insurance.
- Manage an average of 5-8 patients per day, accumulating approximately 6-8 productivity points, with a weekly expectation of 30 points.
- Handle types of care including Start of Care, Wound Care, Follow-up Visits, etc.
- Be willing to travel up to 75 miles from the home clinic, with mileage reimbursed at the IRS standard rate.
- Complete all documentation within 24 hours of patient appointment.
- Accept every patient scheduled, with an understanding that travel may exceed 20 minutes between appointments.
- Participate in floating to a nearby clinic if required to meet financial needs due to low patient census.
- Must be open to weekend and on-call rotations, as needed and discussed during the interview.
- Maintain proficiency in EMR systems employed for patient charting and care management.
