**Job Description:**
- Evaluate and treat patients of all ages, modifying plans of care as appropriate.
- Serve as a role model in delivering professional services and act as a clinical resource for staff and students.
- Perform patient transfers with assistance as needed from bed, chair, and floor, if applicable.
- Demonstrate safe and appropriate body mechanics to reduce the risk of injury to self, staff, and patients.
- Treat patients and colleagues with dignity and respect, remaining empathetic and engaged.
- Design a plan of care that integrates goals, treatment, outcomes, and discharge planning.
- Adjust care based on patient response and available evidence.
- Educate patients, families, caregivers, students, and other healthcare providers using effective teaching methods.
- Evaluate clinical findings to determine physical therapy diagnoses and outcomes of care.
- Perform examinations using reliable and valid methods relevant to the chief complaint, screening results, and patient history.
- Produce documentation in accordance with clinical process policy.
- Screen patients to determine the effectiveness of and need for therapy services.
- Use support personnel according to legal standards and ethical guidelines, monitoring their care delivery and providing regular feedback.
- Evaluate and implement intervention plans utilizing functional activities that consider the functional and emotional needs of the patient and family.
- Modify approach in response to patient behavior and performance.
- Use standardized outcomes measures uniformly to ensure functional performance is accurately reflected.
- Complete documentation in accordance with regulatory and organizational standards.
- Collaborate with patients, families, and significant others throughout the rehabilitation process, providing necessary education and family training.
- Provide supervision to physical therapist assistants and rehabilitation technicians in accordance with regulatory guidelines.
- Clearly communicate patient performance, status changes, activity/transfer recommendations, and discharge disposition and equipment recommendations with the medical team.
