**Job Description:**
- Provide case management and direct patient care within a team environment.
- Administer comprehensive physical therapy modalities in the home setting.
- Assist in evaluating the level of function and help develop and revise the care plan as necessary.
- Prepare clinical and progress notes and consult with the family and other agency personnel.
- Promote organization philosophy, mission, and policies to ensure the delivery of quality care.
- Develop and revise treatment and discharge plans in conjunction with the patient, family, physician, and care delivery team.
- Initiate appropriate rehabilitative programs including gait training, transfers, ADLs, home modification, care, and use of equipment.
- Evaluate patient's progress and make necessary adjustments to the physical therapy program.
- Oversee the quality of patient care services and ensure student health, safety, and compliance with the plan of care.
- Educate patients, families, and team members on disease processes, self-care techniques, and preventive strategies.
- Supervise physical therapy assistants to ensure delivery of quality care.
- Display respect for patients' privacy, confidentiality, and property.
- Interact positively with patients, physicians, referral sources, and other stakeholders.
- Participate in patient conferences and assist in the orientation of new rehabilitation personnel.
- Maintain comprehensive clinical records and refer to appropriate community agencies/resources.
- Demonstrate flexibility, enthusiasm, and willingness to cooperate with others.
