Plans and implements specific treatment programs for Home Health and/or Hospice patients according to the principles and practices of Physical Therapy.
Evaluate patients and establish written plan of treatment with short and/or long term goals.
Completion of an accredited Physical Therapy program
Must have PT licensure in the State of Montana
Must have valid driver's license
BLS Certified
2 years experience
