Position Overview:We are seeking an enthusiastic Physical Therapist Assistant (PTA) to join our team at a Skilled Nursing Facility (SNF) in Media, PA. In this role, you'll work alongside physical therapists to help residents improve mobility, gain independence, and reach their recovery goals.
Responsibilities:

Implement individualized treatment plans under the direction of licensed Physical Therapists.
Assist residents with therapeutic exercises, functional mobility, and other rehabilitation interventions.
Track patient progress and document treatment provided according to facility protocols.
Communicate effectively with the healthcare team and collaborate on treatment modifications.
Provide guidance to residents and their families for continued exercises at home.
Maintain a safe and supportive environment during therapy sessions.
