**Job Title:** Physical Therapist Assistant (PTA) - Home Health

**Experience:** Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience, depending on the specialty. First-timers not accepted; recent specialty experience required.
**License:** Must have license in hand before applying. Pending licenses are not accepted.
**Certifications:** CPR, BLS
**Must-Have:**
- Proficiency with HomeCare HomeBase EMR and recent use within the last 6 months, otherwise, a retraining course is required.
- Must be willing to travel up to 75 miles from home clinic.
- Reliable transportation, valid driver's license, and agency-required liability insurance.
- Open to on-call rotations and weekend requirements discussed during the interview.
- Comfortable with drives that may exceed 20 minutes between patients.
- Ability to complete all documentation within 24 hours of the patient appointment.
**Job Description:**
- Coordinate and direct the provision of home health care according to physician's orders, based on agency policies and procedures.
- Apply the nursing process competently and responsibly to ensure quality care.
- Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Conduct home visits and periodic office meetings.
- Manage a caseload of 5-8 patients per day, totaling a weekly productivity expectation of 30 points.
- Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
- Attend to patients both in-home and in care facilities.
- Expected to accept all patient assignments without declining based on distance unless discussed with management.
- Participate in weekly meetings at the assigned clinic and follow agency protocols efficiently.
