**Guaranteed Hours:** 3 shifts per 13 weeks

**Experience:** 2 years required

**License:** State license required

**Certifications:** BLS certification required

**Must-Have:**
- EPIC charting experience
- Previous home care experience
- Strong critical thinking, assessment, communication, and collaboration skills
- Knowledge in Cardiopulmonary, Geriatrics, Medical Surgical, Orthopedics, Home Health

**Job Description:**

- Provide therapy and rehabilitation services in a home health setting.
- Work collaboratively with Physical Therapists in managing patient care.
- Conduct assessments and manage individualized treatment plans.
- Educate patients and families about home exercise programs and proper use of adaptive devices.
- Evaluate and monitor patient progress, adjusting treatments as needed.
- Manage time efficiently, ensuring all assigned duties are completed within the shift hours.
- Use assistive and adaptive devices, including gait belts, rolling walkers, and sliding boards.
- Conduct pain assessments, strength testing, and skin integrity checks.
- Ensure compliance with all health and safety regulations while providing patient care.
- Handle scheduling of appointments and communication through telephone calls.
- Document patient progress and treatment plans in the EPIC system.
- Manage supplies ordering to maintain the necessary equipment for patient care.
- Maintain professionalism and a clean appearance during home visits.

**Note:** The clinician will cover a territory with a radius of 40-80 miles, and mileage will be reimbursed. Orientation consists of 40 hours.
