**Shift:** Day 3x8-Hour (08:00 - 17:00) - Monday, Wednesday & Friday
**Guaranteed Hours:** 3 shifts per 13 weeks
**Experience:** Minimum 2 years required
**License:** State licensed in Illinois at the time of submission
**Certifications:** Basic Life Support (BLS)

**Job Description:**
- Perform assessments and develop home exercise programs
- Work with cardiopulmonary, geriatrics, medical surgical, and orthopedics cases
- Utilize adaptive devices, AROM/AAROM/PROM, and assistive devices
- Conduct assessments in arousal and attention, pain, sensory response, and skin integrity
- Perform strength testing and vital signs monitoring
- Implement breathing techniques and body mechanics understanding
- Identify environmental barriers and coordinate patient family education
- Handle lifting and transfers, medications, and order supplies
- Schedule appointments and manage telephone calls
- Use gait belts, rolling walkers, sliding boards, and standard walkers
- Work with supplemental oxygen, total hip replacement, and total knee replacement cases
- Charting experience with EPIC required
- Maintain professional appearance with appropriate scrub color/style
- No weekend shifts required
- Territory mileage radius of 40-80 miles with mileage reimbursement
- Flu vaccine required, with medical/religious exemptions only
Preferences are defined for no local candidates within 75-mile radius except specific exempt roles, with specific holiday and return staff policies applicable. Candidates should be willing to float within the service line and facilities if required.
