**Job Title:** Rehabilitation Therapy Position

**Shift:** Day - 4x8 - 7am - 3pm

**Guaranteed Hours:** 32 hours per week

**Experience:** Open to new graduates; Skilled Nursing Facility experience preferred

**License:** State License required; verification needed

**Certifications:** Valid certifications as per state requirements

**Must-Have:**
- Prescreen form filled out and attached with submission
- Resume and license verification attached
- COVID test 72 hours prior to start date with results issued 48 hours prior

**Job Description:**
- Assist physical therapists in providing rehabilitation therapy services.
- Help patients perform prescribed exercises and use equipment.
- Monitor patient progress and report findings to the supervising therapist.
- Ensure the therapy area is clean and organized.
- Facilitate effective communication between patients and the healthcare team.
- Assist with documentation and updating patient records.
- Maintain a respectful and compassionate approach to patient care.
- Complete necessary credentialing documentation, including:
- State License Verification
- Resume
- References (3)
- County and Federal Criminal Background Checks (7 years or more)
- Multi-State Criminal and Nationwide Sex Offender Searches (7 years or more)
- HHS/OIG, GSA, and SAM Searches
- Social Security Number Trace
- 2-Step PPD, Quantiferon, or proof of positive PPD and chest X-ray
- Oregon State Background - CRIMS (facility run)
