**Job Description:**
- Provide physical therapy support and assistance under the supervision of the clinical team.
- Assist patients to achieve optimal functional outcomes during rehabilitation at home.
- Collaborate with a multidisciplinary team to develop and implement individualized care plans.
- Monitor and report patient progress and response to treatment effectively.
- Ensure compliance with medical and safety protocols.
- Participate in patient education and health promotion activities as required.
- Maintain accurate documentation and records in accordance with established procedures.
