**Must-Have:** A completed prescreen form, resume, and license verification must be attached upon submission.

**Job Description:**

- Assist in providing physical therapy services under the supervision of a licensed physical therapist.
- Follow a treatment plan designed by the physical therapist to improve mobility, relieve pain, and prevent or limit permanent physical disabilities.
- Monitor and document patient progress and report findings to the physical therapist.
- Prepare reports on patient progress and communicate effectively with staff and family members.
- Ensure a clean, orderly, and safe environment for patients.
- Participate in departmental meetings and training sessions.
- Conduct COVID testing 72 hours prior to the start date with results issued 48 hours before the start date.
- Complete background checks, including federal and county criminal searches for the past seven years, multi-state criminal search, nationwide sex offender search, HHS/OIG and GSA searches, social security number trace, SAM search, and CORI Massachusetts background check.
- Provide proof of a 1 Step PPD or Quantiferon test or documentation of a positive PPD and chest X-ray.
