**Job Title:** Physical Therapist Assistant (PTA) - Home Health

**Experience:** Must have a minimum of 1 full year Adult Home Health or Hospice experience

**License:** Must have a license in hand

**Certifications:** CPR, BLS

**Must-Have:**
- HomeCare HomeBase (HCHB) proficiency within the last year
- Proficient in Oasis for RNs, PTs, and OTs
- Reliable transportation with current driver's license and liability insurance
- Must be open to traveling up to 75 miles from the assigned care center
- Must be open to on-call rotations if required

**Job Description:**

- Provide, coordinate, and direct home health care according to physicians' orders and agency policies.
- Utilize the nursing process competently.
- Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Conduct home visits and occasional office meetings.
- Manage an average caseload of 5-8 patients per day.
- Assignment points are approximately 6-8 per day, with 30 points required per week from various activities including documentation and travel.
- Types of cases managed include Start of Care, Wound Care, and Follow-up Visits.
- Territory coverage requires travel mileage of 50-200 miles per week, compensated at the IRS Standard rate from patient home to patient home.
- Charting is conducted using the HomeCare HomeBase EMR system.
