- 2 Step PPD, Quantiferon, or Proof of Positive PPD and Chest X-Ray
- SAM Search
- Oregon State Background Check

**Job Description:**
- Ensure prescreen is completed and attached during candidate submission.
- Attach resume and license verification; candidates without these will be declined.
- Conduct a COVID test 72 hours prior to the start date with results ready 48 hours before starting.
- Understand and fulfill all credentialing requirements listed.
