**Job Description:**
- Prepare and execute individualized treatment plans under the supervision of a physical therapist.
- Assist patients in performing therapeutic exercises and functional activities.
- Monitor patient progress and report to the supervising therapist.
- Maintain accurate and up-to-date patient records.
- Conduct relevant assessments and update therapy plans as necessary.
- Adhere to all safety and health protocols, including a COVID test 72 hours prior to start date with results 48 hours prior.
- Participate in departmental meetings and continuing education opportunities.
- Uphold patient confidentiality and adhere to ethical guidelines.
- Complete and verify credentialing checklist, including background checks and multi-state criminal searches.
- Verify Social Security Number trace and provide proof of negative PPD or Quantiferon test or chest X-Ray in case of a positive PPD.
