Job Description:
- Ensure all credentialing requirements are met and attached upon submission.
- Conduct patient assessments to determine suitable physical therapy treatments.
- Develop personalized care plans to address patient needs.
- Monitor patient progress and adjust treatments as necessary.
- Collaborate with a multidisciplinary team to provide comprehensive patient care.
- Maintain accurate patient records and documentation.
- Participate in team meetings and contribute to patient care decisions.
- Ensure compliance with all regulations and standards.
- Maintain patient confidentiality and professional conduct.

Credentialing Checklist:
