- Completion of a prescreen
- Attached resume and license verification
- Covid test completed 72 hours prior to start date, with results available 48 hours prior

**Job Description:**

- Ensure prescreen is filled out and attached when submitting a candidate.
- Include a resume, license verification, and references (3) during submission; otherwise, the candidate will be declined.
- Conduct a background check, including county, federal, multi-state criminal search, and nationwide sex offender search (all for 7 years or more).
- Perform HHS/OIG and GSA searches.
- Conduct Social Security Number Trace and SAM Search.
- Provide 2 Step PPD, Quantiferon, or proof of positive PPD and chest X-Ray.
- Oregon State Background Check to be completed (facility-run).
- No requirement for bilingual or holiday coverage.
