**Job Description:**
- Conduct physical therapy sessions under the supervision of licensed physical therapists.
- Assist patients in exercises and therapy activities designed to improve their physical wellbeing.
- Maintain patient records and document progress accurately.
- Communicate effectively with patients, their families, and the rehabilitation team.
- Prepare treatment areas and ensure equipment is in working order.
- Follow COVID-19 testing protocols, including a test 72 hours before start date, with results obtained 48 hours prior.
- Complete necessary background checks, including:
- References (3)
- County Criminal Check (7 years for all counties lived and worked)
- Federal Criminal Check (7 years or more)
- Multi-State Criminal Search (7 years or more)
- Nationwide Sex Offender Search (7 years or more)
- HHS/OIG Search
- GSA Search
- Social Security Number Trace
- 2 Step PPD or Quantiferon or proof of positive PPD and Chest X-Ray
- SAM Search
- Oregon State Background - CRIMS (facility run)
