**Job Description:**

- Provide high-quality physical therapy services to patients in both SNF and LTACH settings.
- Collaborate with a multidisciplinary team to develop and implement individualized patient care plans.
- Assist with the rehabilitation of patients, aiding them in regaining physical function and improving mobility.
- Document patient progress and maintain accurate records in accordance with facility standards.
- Adhere to facility dress code, wearing scrubs of any color.
- Contribute to a positive team environment, offering flexibility and support to other units as needed.
