**Shift:** Day 5 x 8-Hour (08:00 - 17:00)

**Guaranteed Hours:** 3 shifts per 13 weeks

**Experience:** Minimum of 1 year required

**License:** Valid state license required at the time of submission

**Certifications:** Basic Life Support (BLS) required

**Must-Have:**
- Skills in Adaptive Devices, AROM/AAROM/PROM, Arousal and Attention, Assistive Devices, etc.
- Experience with Dementia/Alzheimer's, Cognitive disorders, and Home Exercise Programs.
- Proficiency in Gait training and handling equipment like Gait belts, Hoyer lifts, Rolling Walkers.

**Job Description:**

- Conduct 1:6 patient visits per day in a home health setting, focusing on mainly adult and geriatric care.
- Perform comprehensive pain assessments, sensory response evaluations, and strength testing.
- Utilize assistive and adaptive devices to enhance patient mobility and functionality.
- Identify and address environmental barriers within the home setting.
- Educate and collaborate with patients and families for effective home exercise programs and transfers.
- Coordinate with support physical therapists to ensure team-based patient care.
- Ensure skin integrity and vital signs monitoring to manage patient health status.
- Utilize Epic for charting and maintain detailed patient records.
- Provide weekend service on-call as required (not mandatory).
- Travel within a territory mileage radius of 40-80 miles, with mileage reimbursed.
- Participate in orientation for 40 hours, non-billable.
- Engage in continuing education and skills training as required through service line modules.
