**Job Title:** Physical Therapist Assistant in Home Health

**Shift:** Day 5x8-Hour (08:00 - 17:00)
**Guaranteed Hours:** 3 shifts per 13 weeks
**Experience:** Minimum 1 year preferred; First-timers accepted
**License:** Valid State License required at time of submission
**Certifications:** BLS Certification required
**Must-Have:**
- Experience with Adaptive Devices
- Skills in AROM/AAROM/PROM
- Knowledge of Assistive Devices, Body Mechanics, Pain Assessment
- Ability in Gait Training and Transfer Techniques
**Job Description:**
- Work primarily with adults and geriatric patients in a home health setting
- Evaluate and assist patients using adaptive and assistive devices
- Conduct strength testing, vital signs monitoring, and gait training
- Provide and instruct patients and families on Home Exercise Programs
- Assess environmental barriers and make recommendations for accommodations
- Conduct sensory response and skin integrity assessments
- Manage cognitive disorders and conditions like Dementia/Alzheimer's
- Participate in patient family education and maintain clear communication
- Work collaboratively with support Physical Therapist to deliver patient care
- Perform patient lifting and transfers utilizing gait belts, Hoyer lifts, and other equipment
- Complete necessary charting via EPIC system
**Special Requirements:**
- Must be willing to travel within a 40-80 mile radius, mileage reimbursed
- Work five days a week with no weekend requirement
- Complete orientation and compliance modules before the start date
- Holiday work requirement: must work two of these three holidays - Thanksgiving, Christmas, and New Year's Day
- Must comply with the latest vaccination requirements
Additional notes: Background checks and references are required. Interest in extending contract beyond 13 weeks or preference for 36 or 48 work hours per week should be indicated.
