**Job Title:** Speech-Language Pathologist (SLP) - Transitional Care

**Job Specialty:** Therapy/Rehabilitation - Skilled Nursing Facility (SNF)

**Job Duration:** 13 weeks

**Shift:** Day 5x8-Hour (08:00 - 16:30, Monday - Friday)

**Guaranteed Hours:** Facility may call off traveler up to 1 shift per pay period (every 2 weeks)

**Experience:** 1 year of skilled nursing experience required

**License:** License required at time of signature. Must have license in hand.

**Certifications:** BLS

**Must-Have:**
- Experience with dysphagia management (bedside swallow evaluation, compensatory strategies, hyolaryngeal exercises)
- Skills in plan of care, assessment/evaluation/discharge
- Reliable transportation and no additional scheduling restrictions

**Job Description:**

- Provide speech-language pathology services in a transitional care setting.
- Maintain a 1:6-8 patient ratio.
- Work schedule is Monday through Friday, with no weekend requirement.
- Complete 40 hours of orientation.
- Utilize Epic for charting.
- Adhere to facility's scrub color policies (any basic color scrubs).
- Comply with facility policies, including no time off allowed around holidays.
- First-time travelers are accepted and locals are welcome within a 50-mile radius.
- Participation in modules typically requires 16 hours, with completion recorded in Workramp transcripts.
- Travelers are expected to float between all facilities within the state if required.
- Ensure proper completion of all duties without on-call requirements.
