**Must-Have:** Resume, license verification, and prescreen filled out and attached upon submission

**Job Description:**

- Complete and attach the prescreen, resume, and license verification upon candidate submission.
- Covid testing required 72 hours prior to the start date with results issued 48 hours prior.
- Conduct thorough criminal background checks covering County, Federal, Multi-State Criminal Search, and Nationwide Sex Offender Search for the past 7 years or more.
- Perform HHS/OIG and GSA Searches.
- Complete Social Security Number Trace.
- Ensure 1 Step PPD or Quantiferon testing or provide proof of positive PPD with Chest X-Ray.
- Execute SAM Search and CORI Massachusetts Background checks.
- Provide three professional references.
- Deliver 8 hours of Dementia Training.

This position requires attention to detail and adherence to credentialing requirements to ensure seamless onboarding.
