**Job Description:**
- Effectively screen, evaluate, and rehabilitate patients with communication, cognitive, or swallowing disorders.
- Develop appropriate plans of care following regulatory and clinical practice standards.
- Utilize standardized assessments and evidence-based practices to support clinical interventions.
- Assess patient progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
- Evaluate treatment outcomes for effectiveness, modifying services as needed and making referrals related to outcomes.
- Provide consultation and counseling to patients, families, caregivers, and other service providers related to speech disorders.
- Communicate effectively with supervisors and other health team members regarding patient progress, barriers, and treatment plans.
- Provide comprehensive discharge summaries of services provided per regulatory and clinical requirements.
- Document all regulatory and clinically required information in the electronic documentation system and maintain patient records according to company policy.
- Accurately record treatment minutes for services provided per regulatory and clinical requirements.
- Participate in required meetings, including patient care conferences, utilization review meetings, and rehabilitation meetings.
- Participate in facility and company-required training, in-services, and conferences.
- Obtain any necessary DME or supplies per regulatory and clinical practice requirements.
- Supervise and support CFY or Student Speech-Language Pathologists.
- Adhere to ASHA and state-required practice acts, code of conduct, and ethics during professional practice as a Speech-Language Pathologist.
