- Medicare-certified home health agency experience

**Job Description:**

- Provide speech-language therapy services for adult and geriatric patients in a home health setting.
- Manage a caseload of 5-6 patients per day, achieving 27 points per day/week.
- Conduct evaluations, develop plans of care, and perform reassessments.
- Document Medicare documentation using Pointcare/HCHB (Homecare Homebase).
- Educate family members on patient care and therapy techniques.
- Conduct bedside swallow evaluations and use of thickening agents.
- Implement compensatory techniques and therapy for CVA patients.
- Perform discharge planning and telephone consultations.
- Provide care for patients with multiple conditions including Multiple Sclerosis, TBI, degenerative diseases, anoxia, laryngectomy, and tracheotomy.
- Experience with OASIS E, Adaptive Equipment, and Transfer Training preferred.
- Must adhere to dress code by wearing navy blue scrubs.
- Compliance with flu vaccine requirements is mandatory.
