- Bachelor's and Master's Degree in Speech-Language Pathology
- Graduate of an accredited school of speech language pathology
**Job Description:**
- Work across outpatient, inpatient, and home health/hospice disciplines to provide speech language pathology services.
- Collaborate with a team that includes occupational and physical therapy services, nursing, and home health aides.
- Work one-on-one with patients, ensuring time for documentation in schedule.
- Use a fleet of vehicles available for home health/hospice services when needed.
- Encourage continuing education with annual stipends and trainings integrated into schedules.
- Participate in an interdisciplinary team approach for patient care.
- Provide treatment to improve speech/language function, cognition, breathing, swallowing, etc., as per physician's orders.
- Coordinate therapeutic activities with patients, caregivers, and other health care disciplines.
- Evaluate patient communication, cognitive, and physical functions to determine levels of self-care and independence.
- Develop treatment plans involving patients, families, and social support systems.
- Ensure timely and complete documentation in line with compliance requirements.
- Coordinate services and ensure follow-up and continuing care as needed.
- Participate in quality and performance improvement activities within the department.
- Supervise students and support personnel while adhering to professional standards.
- Maintain effective communication in both written and oral forms.
- Adhere to relevant regulatory requirements and hospital standards.
- Demonstrate excellent judgment, discretion, and decision-making abilities.
- Promote a culture of safety, quality, and positive patient experience.
