**Job Description:**
- Effectively screen, evaluate, and rehabilitate patients with communication, cognitive, or swallowing disorders.
- Develop and implement appropriate plans of care following all regulatory and clinical practice standards.
- Utilize standardized assessments and evidence-based practices to support clinical interventions.
- Assess patient progress and adjust treatments as necessary, documenting changes per regulatory and clinical practice requirements.
- Evaluate treatment outcomes for effectiveness and modify services based on patient evaluation.
- Provide consultation and counseling to patients, families, caregivers, and other service providers related to speech disorders.
- Communicate effectively with supervisors and other health team members regarding patient progress, barriers, and treatment plans.
- Provide a comprehensive discharge summary of services as per regulatory and clinical requirements.
- Document all required regulatory and clinical information in the electronic documentation system.
- Accurately record treatment minutes for services provided as per regulatory and clinical requirements.
- Participate in required meetings including patient care conferences, utilization review meetings, and rehabilitation meetings.
- Engage in facility and company-required training, in-services, and conferences.
- Obtain necessary Durable Medical Equipment (DME) or supplies as per regulatory and clinical practice requirements.
- Supervise and support Clinical Fellowship Year (CFY) or Student Speech-Language Pathologists.
- Adhere to ASHA and State Required practice acts, code of conduct, and ethics during professional practice.
