- Provide speech therapy services for home health patients with critical needs.
- Develop and implement individualized treatment plans tailored to patient needs.
- Conduct assessments to evaluate speech, language, cognitive-communication, and swallowing disorders.
- Monitor patient progress and adjust treatment plans accordingly.
- Work collaboratively with an interdisciplinary team to ensure comprehensive patient care.
- Educate patients and families about communication disorders and therapeutic interventions.
- Maintain accurate and timely documentation of patient assessments, treatments, and progress.
