**Certifications:** CPR (fundamental facilities)

**Must-Have:**
- Complete a prescreening that must be completed and attached upon submission
- Attach a resume and license verification

**Job Description:**
- Conduct a State License Verification
- Provide a resume and three references
- Complete County Criminal check (7 years for all counties lived and worked)
- Complete Federal Criminal check (7 years or more)
- Perform Multi-State Criminal Search (7 years or more)
- Conduct Nationwide Sex Offender Search (7 years or more)
- Complete HHS/OIG and GSA Search
- Conduct a Social Security Number Trace
- Provide 1 Step PPD or Quantiferon or Proof of Positive PPD and Chest X-Ray
- Complete SAM Search
- Negativity confirmed through Drug Screen Results
- Submit Covid test results 72 hours prior to the start date, with results issued 48 hours prior
