Job Description

The Certified Clinically Advanced Nutrition Coordinator (CCANC) is responsible for activities that support patient focused care within their clinical specialty area and adheres to department and hospital philosophies, standards, objectives and policies and procedures.
The CCANC is an expert in their specialty and acts a resource to both the medical and clinical nutrition staff.
The CCANC identifies and collects performance improvement data and coordinates activities for the dietetic interns.
This individual assists with administrative duties and provides coverage for the Director of Clinical Nutrition in their absence.

Responsibilities

1.
Evaluates the nutritional status of patients by reviewing pertinent information obtained from the electronic medical record and the interdisciplinary healthcare team.
Applies knowledge of nutrition screening and assessment to specific patient populations to determine their nutritional needs.
Conducts nutrition-focused physical exams.

2.
Possesses population-specific skill set and advanced credential in clinical nutrition assessment.

3.
Documents pertinent findings in the medical record per policy.

4.
Provides quality nutrition counseling to patients and family members as indicated.

5.
Evaluates nutrition care plans for effectiveness and recommends changes as needed.
Uses professional judgment to adapt and apply guidelines, protocols and professional standards of care to the new and changing needs of the patient.

6.
Acts as a key member of the interdisciplinary team by participating in the multidisciplinary approach to patient care.
May coordinate rounds in specific service areas.

7.
Evaluates the need for enteral and parenteral nutrition support.
Assists with recommendations for nutrition support per hospital and department policy.

8.
May coordinate and manage activities for the dietetic interns rotating through the department from various programs and universities.

9.
Coordinates research activities for the department.

10.
Coordinates quality improvement initiatives for the department.

11.
Participates in the recruitment, hiring, training, and development of department staff.

12.
Participates in the evaluation and disciplinary processes of department staff.

13.
Organizes projects and activities within the department, including inservices, interdepartmental/community education programs, journal clubs, and lectures.

14.
Organizes the development and review of nutrition education materials.

15.
Other duties as assigned.

Qualifications Bachelors degree or higher from an accredited institution, with completion of ACEND (Accreditation Council for Education in Nutrition & Dietetics)-accredited dietetic internship Masters Degree preferred 5+ years of clinical experience, including 2-3 years in specialty area Research experience preferred Registered Dietitian (RD) from the Commission on Dietetic Registration (CDR) required Certified Dietitian Nutritionist (CDN) from the NYS Department of Education Office of the Professions preferred Advanced Certification, i.e.
Certified Diabetes Educator (CDE), Certified Nutrition Support Clinician (CNSC), and/or Pediatric/Renal/Oncology Specialty required About Us Strength Through Diversity
