Lumibility is a person-centered agency serving all individuals with intellectual and/or developmental delays.
We provide in-home, day support, transitional, and employment services to adults ages 21 and older, and have been a leading provider of human services throughout various towns in Connecticut since 1957.

The employment specialist-Per diem will provide direct support service to people with differing abilities served by Lumibility in competitive and supported employment and/or volunteer sites.
The Job Coach is responsible for assisting in learning specific job tasks, job-related attitudes, and worksite culture.
Implement employment supports that are person centered and promote positive outcomes.
Provide the environment and supports necessary to promote appropriate social and vocational success, as well as positive behaviors that will enhance skills, enrich lives, and foster community presence & participation.


Pay Rate: $21/hour

Hours: M-F Per Diem, between 8am-6pm

Essential Functions (not limited to):

•Ensure the health, safety and well-being of people served by Lumibility in accordance with the individual service plan and in compliance with agency policies and procedures.


•Provide transportation for assigned service individuals in accordance with Lumibility safe-driving policies and practices.


•Teach and model skills necessary to perform job duties.


•Coach individuals on expected work attire and grooming.


•Advocates for individuals to employers or coworkers, identifying the individual's strengths, accommodations, and needs, when/if needed.


•Help individuals establish appropriate relationships with coworkers and employers.


•Act as liaison with the employer and coworkers to establish ongoing worksite support.


•With individual and team input, assist in the development of goals and objectives for ongoing placement success and retention.


•Regularly meet with the employer and update the individual's employment information such as job description, job tasks, salary, benefits, Union information (if applicable), and performance reviews.


•Regularly communicate progress and employment issues to the individual's support team enlisting support from interdisciplinary team members for support.


•Completes progress notes and reports and other documentation as required.
Documents consistently and accurately all activities and outcomes.


•Represent the agency in a professional and positive light at all community partners.


•Develop and maintain positive relationships with service individuals, families, co-workers and other community representatives.


•Other duties, assignments and work locations as assigned.


Requirements


Knowledge of person-centered, outcome-based services for people with differing abilities Knowledge of company policies and procedures and applicable regulatory standards Knowledge of community resources Skill in working with people with differing abilities Skill in operating standard office equipment, computer and various software packages.
Skill in implementing programs & activities as outlined in the individual support plan Skill in communicating effectively in a courteous and professional manner Skill in writing clearly and concisely to maintain accurate records Ability to attend to the physical and personal needs of adults with differing abilities Ability to demonstrate good communication and interpersonal skills necessary to provide guidance and instruction to adults with significant challenges; Ability to work successfully, both independently and in a team environment Ability to problem-solve effectively Ability to maintain confidentiality Ability to use adaptive and other equipment Ability to maintain a patient and positive attitude Ability to drive and travel to varying locations as needed REQUIRED PHYSICAL EFFORT: Must be able to sit, stand and walk at an assigned location(s) and work for extended periods of time; bend/stoop, kneel and balance frequently; climb, reach above shoulder level; push/pull, squat and crouch occasionally; with assistance, lift, carry and position individuals of various weight.
Must be able to lift and carry up to 50 pounds occasionally.
Must be capable of driving multi-passenger vehicle and/or personal vehicle for extended periods throughout the day.
