JOB SUMMARY: The Field Intake Nurse is responsible for assessing a member's eligibility for the Managed Long Term Care program. The Intake Nurse will conduct a face-to-face comprehensive assessment utilizing NYSDOH approved assessment tool (UASNY) with a potential member in his/her place of residence. The Intake RN also utilizes CPHL's database of record, completing all necessary documentation to meet CPHL standards. This position requires thorough knowledge of CPHL's program and services, and the ability to communicate clearly the member benefits. The Intake Nurse must possess clinical skills and good judgment, implementing safe plans of care, along with the ability to empathically discuss and describe CPHL services and regulations.
PRIMARY RESPONSIBILITIES: Assesses prospects in their place of residence, completing a comprehensive assessment (UASNY) to determine eligibility and initial plan of care.Effectively completes the required assessment form (UASNY) and other CPHL necessary documents within the required timeframes and in accordance with CPHL established policies and procedures and State and Federal guidelines and regulations.Communicates plan benefits clearly and easily to prospects and referral sources.Completes and submits thorough and high-quality documentation in a timely manner.Appropriately alerts Supervisors regarding any issues encountered in the field.Recognizes quality of care issues and escalates issues appropriately, communicating effectively with coordinators and supervisors as needed.Complies with all CPHL's standards for documentation.Maintains member confidentiality and compliance with HIPAA requirements.Participates in regularly scheduled and other staff meetings as requested.Participates in quality improvement initiatives.Other duties as assigned.EDUCATION AND EXPERIENCE: Education: Graduate of a 2-4 year accredited nursing program.Active and unrestricted NYS RN license.Type of Experience: Specific Technical Skills and Knowledge: Strong written and verbal communication skills.Proficient in the use of Microsoft Office Suite tools.Ease with learning new database systems.Certifications/Licensure: Active and unrestricted NYS RN license.Must possess a valid driver's license, motor vehicle insurance, and access to a motor vehicle, or access to reliable public transportation within the appropriate service area.Ability to travel within assigned area.Professional Liability Insurance.SCOPE INFORMATION: # Direct Reports: 0
